To schedule an appointment, please call during business hours at the specific health center at which you wish to be seen.

The Office Manager at the clinic at which you are a patient will set up your username and password to access the patient portal. Our patient portal can be accessed at http://portal.aqyjhdb.com. Simply click on this link and login by entering your individual username and password to access your Electronic Health Records (EHR).
